ABSTRACT

ABSTRACT We report two cases of severe thermal burns on the ocular surface and its adnexal appendages that developed secondary to exposure to molten heavy metal with a melting temperature of near-thousand degree Celsius. Despite aggressive intervention and strict monitoring, the profound inflammation caused significant damage to the ocular surface, ending up in an intractable infection with an unfavorable outcome. The heat of the molten metal at impact, the heat-retaining capacity of the heavy metal, the total area of the ocular surface exposed to the molten metal, and the duration of exposure determined the severity of the injury. The unfavorable outcome, despite an intensive treatment, in terms of visual acuity and cosmetic appearance, should be explicitly explained to the patient, and a psychiatrist consultation should be considered if necessary.


RESUMO Relatamos dois casos de queimaduras graves da superfície ocular e seus anexos causadas por exposição à metais pesados derretidos com temperatura de fusão de quase mil graus Celsius. Embora rigorosamente acompanhados, a inflamação intensa causou danos significativos para a superfície ocular, que acabou em uma infecção intratável com um resultado desfavorável. O calor do metal fundido no momento do impacto, a capacidade de retenção de calor do metal pesado, a área total da superfície ocular exposta ao metal fundido e a duração da exposição determinaram a gravidade da lesão que vai ocorrer. O resultado desfavorável, apesar do tratamento intensivo, em relação à acuidade visual e à aparência estética, devem ser claramente explicados ao paciente e uma consulta ao psiquiatra deve ser considerada, se necessário.

En el presente caso se describe quistes maculares bilaterales secundarios a descarga eléctrica, los cuales fueron diagnosticados por medio de una Tomografía Óptica Coherente y donde se evidencio el diagnóstico diferencial entre quistes maculares y agujeros maculares siendo la OCT una útil herramienta en este caso.

ABSTRACT

Forty cases of ocular trauma in children were reviewed. The age of the patients ranged from 2 years to 12 years with a mean of 8 years and 6 months. 32 cases. [80%] needed surgical management and 8 cases 120%] were managed conservatively. Patients of penetrating ocular injuries [Group 1] had poor visual prognosis 6/18 or worse, as compared to blunt ocular trauma [Group 2] and ocular burns [Group 3] 6/6-6/18. Injuries involving the lens in the anterior segment or the posterior segment were associated with afar worse prognosis due to the problems of maintenance of binocular single vision. Children under 6 years had poor visual prognosis due to development of amblyopia. Education of the parents for adopting preventive measures at home and at the place of play would save these children as well as other children from ocular injuries in future

ABSTRACT

Las quemaduras corneales pueden presentarse por varios agentes químicos tanto ácidos como alcalinos o por agentes térmicos, las lesiones microscópicas se pueden valorar por los signos clínicos y la clasificiación de Huges-Roper-Hall, pero las lesiones microscópicas de los tejidos, en especial del endotelio corneal, no son valorables por este medio, por lo que la microscopía especular sería muy útil en estos casos. Se incluyeron a todos los pacientes con quemaduras unioculares, se les realizó microscopía especular en ambos ojos y se relacionó el grado de quemadura con las alteraciones endoteliales encontradas. Los cambios celulares encontrados fueron disminución de la densidad celular que es directamente proporcional a la disminución del porcentaje de hexagonalidad y esto a su vez presentó un agrandamiento celular y espacios intercelulares amplios
